ABL Wellness Clinic offers Direct Primary Care (DPC) memberships, bioidentical hormone therapy, and metabolic health services with clear, upfront pricing. DPC is a medical service, not health insurance. We recommend maintaining a high-deductible insurance plan with an HSA to cover hospitalizations, specialist care, surgery, and imaging. DPC fees may generally be paid with HSA funds when allowed under current regulations.
We see patients starting at age 16 and older. In select cases, we may see pediatric urgent care patients starting at age 4.
Provider availability note: Because Dr. Guy is establishing this practice, he also works in an outside emergency department once or twice per month, usually on weekends. When out of town, he remains available to members via telemedicine, phone calls, and text messaging. In-person visits are typically scheduled Monday afternoons through Thursday afternoons, with his work schedule posted on this website.
Membership with ABL Wellness Clinic does not constitute insurance. I do recommend that you a high deductible plan with an HAS (health savings account) have an insurance plan that covers hospital care, accidents, catastrophic diagnoses, specialist care, radiology, and more advanced services as deemed appropriate. You can pay for your membership with your HSA account making membership deductible on your taxes.
Please note that if you do have insurance, we do not bill insurance and therefore you are responsible for all forms to be filled out, whether or not your insurance may cover any studies I may order. You of course do not need to be enrolled in insurance to be a member at ABL Wellness Clinic. The below prices include management of your chronic medical conditions and any urgent care that may arise during your membership.

Please note that because Dr. Guy is just starting this business, he works at an outside ER, usually during the weekends, once or twice per month. During those times he is out of town he will accommodate you via telemedicine/ phone calls/ and texts. He is usually in town on Mondays through Thursdays and some weekends. You can find his work schedule on this website. Visits will be scheduled Monday afternoons through Thursday afternoons.
Best for: Budget-conscious adults with 1–2 chronic medical conditions who want ongoing primary care and urgent care access.
Add-on family member: $69 / month each.

Best for: Patients with 3 or more chronic medical conditions or those who are overweight/obese with multiple medical issues who need closer follow-up.

Prices vary based on type of hormones, whether they are creams, pellets, or injections, and based on duration of application. The laboratory is important as it helps dictate initial treatment and effectiveness of maintenance treatment. Except for The Gold Primary Care Plan (Concierge), any questions should be directed to the staff at ABL who will message the doctor. Additionally, you have opportunity to discuss any questions during your initial consultation as well as the phone/telemedicine visit after the 3-month laboratory results are available.
If you are on a Silver or Gold plan for Primary Care and you want to add hormones to your regimen, then you will simply pay for the creams at our cost of $73 per 30grams (2 months at dose of 0.5 grams per day) container and $133 per 90 grams (6 months at a dose of 0.5 grams per day) container. If you are on the bronze primary care plan you will need to change to the Hormone Plan for primary Care which is $129/month.
Ideal if you may switch to pellets after trying creams for 2 months.

After dosing is established, before each pelleting cycle, labs needed:
PSA, CBC, Testosterone, Estradiol prior to each cycle.
Once pellets are placed, they cannot be removed.
Pellets last around 4–6 months, depending on:
Exercise level
Metabolic rate
Pellet type
We use pellets comparable to BioTE, supplied by Carrie Boyd Pharmaceuticals, a reputable company.
Results vary by individual; success is not guaranteed.
If you are unsure, you may prefer starting with creams on a 2-month plan before committing to pellets.
The procedure is typically not very painful, but:
Pellets may not always stay inserted
You may experience post-procedure discomfort
Cutaneous nerves can be irritated
A small lump at the site is possible and may last a few months, especially with higher testosterone doses (e.g., >1000 mg).
For the first 24 hours:
Keep the bandage clean and dry
Avoid bathing, swimming, or soaking
For 3–5 days:
Avoid:
Heavy lower-body exercise
Running, squats, lunges
Cycling or rowing
Core twisting
Walking is okay. Avoid direct pressure on the pellet site (glute/hip).
For 7 days:
No swimming, hot tubs, saunas, or baths
No submersion of the incision area
Wound Care:
Leave the pressure dressing on for 24 hours
After 24 hours, remove outer dressing, leave steri-strips in place
Steri-strips usually fall off on their own within 5–7 days
Keep the area clean and dry
Do not apply creams or ointments over the site
If you have your own primary care physician and only need hormone prescriptions, you may use our non-member hormone cream program.

Important:
Women can overshoot male testosterone ranges even with small injectable doses, increasing the risk of side effects and irregular bleeding if the uterus is present. Risks increase with high peak levels.

Pay for Hormone Creams
You pay the 2-month or 6-month cream cost up front, as per your chosen plan.
We charge only what we pay the compounding pharmacy plus a $13 processing fee.
Dr. Guy calls the pharmacy; creams are shipped to your home in 2–3 days.
Start Hormone Therapy
Typically 0.5 grams (1 click) per day per cream, unless otherwise directed.
Three-Month Follow-Up Labs
After 3 months, you receive an email link to discounted follow-up labs (~$53.40 plus a small draw fee).
We check estradiol/testosterone/progesterone levels as appropriate.
You will be contacted by Dr. Guy or staff with results and recommendations.
You can also view your results in the patient portal.
Ongoing Care & Refills
As a Primary Care member, you may schedule visits with Dr. Guy or his staff anytime.
For refills, call the office; we will coordinate with the pharmacy.
Labs are billed separately, though at heavily discounted prices.
Creams must be paid up front for the 2-month or 6-month supply.
After initial setup and labs, your ongoing recurring cost is typically the $129/month membership (plus future labs/medications as needed).
There are no returns on purchased hormone creams.
Hormone depletion can reflect primary or secondary conditions (e.g., pituitary, adrenal, or ovarian tumors), not just menopause or andropause.
The doctor’s job is to evaluate all possible causes of low hormone states and treat you safely and comprehensively.
